Minor technical errors are the most common reason documents are sent back. Players should use a properly illuminated, flat surface and avoid flash reflections that hide text. All four corners of the document must be visible, and nothing should be cropped out. File size limits are important: large images may fail to upload, while heavily compressed files can become unreadable.
